Patient participation group
We are setting up a Patient Participation Group and are asking those
who may be interested to take a few moments to complete the following
demographic and interest survey.
To ensure that we have good representation of the practice patients
within the group, the questions are designed to focus on specific areas.
At this stage we do not know how many patients we require. Please return
the completed survey to the surgery and let us know your thoughts.
