University request for
medical report

Student sick notes

WE DO NOT ISSUE SICK NOTES FOR ILLNESS
LASTING FIVE DAYS OR LESS.

University regulations state that illness lasting:

• 0 - 5 days Does not require a doctor's certificate
Use university self-certification form instead
• 6+ days Requires medical certification
See your doctor to request certification

If your illness lasts five days or fewer please complete a self-certification form, which can be obtained from your faculty office or downloaded here.

This remains the case even if an exam will be or was affected by a short-term illness.

Medical certification relates only to prospective certification - i.e. an illness that you are suffering at the moment and that will continue to affect you in the future in such a way that it will fully prevent your attendance at university.

Medical reports

If you believe your academic performance has been affected by:
    - one or more illnesses in the past
    - an on-going problem
    - a problem(s) which was not sufficient to completely prevent
      your attendance at university

and you are seeking to:
     - apply for an extension to a deadline
     - apply for special considerations
     - simply wish to inform your department about a medical matter

then you should follow the university procedure for this process:

1 Discuss your problem with your personal tutor (or other department representative) that can help with the problem
2 If you both agree that there are suitable grounds then both the student and tutor should complete the university form requesting a medical report
3 Deliver your completed report request form to our reception and pay the private services fee
4 We will endeavour to prepare your report within five working days
5 Collect the report from reception

You can download the university report request form here or obtain it from your department.

If you attempt to bypass this process by simply booking an appointment to get a "note" or a "letter", please expect to be sent away by the doctor to complete the above procedure (as set out by the university).
