New patient registration
We register patients who live within the following
postcodes only. Please use your home postcode
in Southampton (e.g. SO12 3AB) to determine your eligibility to
register with us.

come in to the surgery. You will be asked to complete a medical
information form (which you can
download here and complete before arrival), we will create new
computer records and request the transfer of your notes from your
previous GP. This process needs to be completed in
person. If you have an NHS card, please bring it along with you too. |
