Welcome to Southampton!

Hi! Our aim here at the University Health Service is to keep in you in good health and help you have an enjoyable and successful time at university.

Medical registration with the practice is on the following dates:

You will find a copy of our medical information form in you hall welcome pack when you arrive, but you can also download a copy here if you wish to complete it at home before you leave.

Please:
   • fill this in before you arrive to register,
   • use black pen and block capitals,
   • give your Southampton address,
   • complete all the medical information requested.

This form can be used to register at either of the campus practices (the University Health Service or Highfield Health) but your registration will only be valid at the practice you registered with.
